About Homemaker Personal care

At Health Care Revolution LLC, we ensure that individuals with intellectual and developmental disabilities lead healthy, happy lives, no matter if they live independently, with family or with other individuals.  Our In-home Homemaker and Personal Care Services bring members of our caring staff into an adult’s home to offer the following:

• Homemaker assistance
• Personal care
• Scheduled and emergency respite
• Transportation coordination
• Other individualized supports as needed

Our personalized services help adults with intellectual and developmental disabilities accomplish their daily activities, learn new skills and live as independently as possible in their own home.

We get to know each person in the program and design services that leverage their strengths and help them know what to expect no matter the challenges or activities in a given day.

Whether it’s cleaning, cooking, maintaining good hygiene, or walking to the store, the individuals in our In-Home Homemaker and Personal Care Services program learn at a pace that’s comfortable for them so they can build confidence for the future.

Flexible, Personalized Program

We offer support for adults with intellectual and developmental disabilities and can assist you in linkage to funding sources and payment options.
