Non-Medical Transportation (NMT)

Homemaker Personal Care (HPC) Transportation
Our Homemaker/Personal Care is provided to help a person with daily living activities such as personal hygiene, dressing and eating.
Waiver Nursing Delegation (WND)
Through this service, you or your loved one gets to receive healthcare assistance right where you live. Registered nurses assess your condition and determine the kind of aid you need with your doctor’s advice.
Participant Directed HPC
Health Care Revolution LLC is licensed by the Department of Developmental Disabilities to provide participant directed HPC services. This gives individuals enrolled in a waiver program more control and authority to decide who provides needed services and how those services are provided.
